Is individual health insurance coverage guaranteed?

No.  Many times when you apply for individual health insurance you will be subject to medical underwriting.  If the insurance carrier does not have enough information from the application, they can either request a phone interview to gather more information or even request a physical exam.  Make sure you research all the carriers, such as Anthem Blue Cross of California and Blue Shield of California, to find the right plan for you.
